FIRST VISIT OFFICE GUIDE

This guide outlines the essential steps for preparing for and conducting a new patient's first appointment. The primary goal is to ensure a smooth, comprehensive, and positive experience, laying the groundwork for ongoing care.

Bring a Support Person
 

  • You’re welcome to bring a family member. They can help:

  • Listen and take notes

  • Remember symptoms or episodes

  • Provide comfort and support

  • Heart health involves your whole support system, and we want you to feel informed and supported throughout your visit.
     

Arrive Early
 

  • We recommend arriving 15–20 minutes before your scheduled time so you can:

  • Complete new patient paperwork

  • Ensure we’ve received prior test results or medical records

  • Update insurance or personal information

  • Settle in without feeling rushed

  • A typical first cardiology visit lasts around 40 minutes.
     

What Information Your Cardiologist Will Ask
 

To help us understand your heart health, please be ready to provide information about the following:
 

1. Your Symptoms
 

If you are experiencing symptoms, we will ask:
 

  • What the symptoms are

  • How often they occur

  • What triggers or relieves them
     

2. Personal & Family Medical History
 

Our team needs a full picture of your health, including:
 

  • Current medical conditions

  • Allergies

  • Past surgeries or hospitalizations

  • Medications you take

  • Family history of heart disease and related conditions
     

3. Medications
 

Bring one of the following:

  • A complete list of all medications (including supplements and over-the-counter drugs)
    or

  • All medication bottles/containers
    ​

If you cannot bring your medication list or bottles on the day of your visit, you may fax them to us ahead of time at:

4. Lifestyle Habits
 

Your provider may ask about:

​

  • Exercise routines

  • Diet and nutrition

  • Smoking history

  • Alcohol or drug use

  • Sleep and stress management
     

These help us understand how your daily habits may influence your heart health.
 

5. Previous Records & Test Results
 

If you have had previous cardiac tests or consultations, please bring:

​

  • EKGs

  • Echocardiograms

  • Stress tests

  • Bloodwork

  • Hospital or ER records
     

If you’re unable to bring physical records, please ask your previous provider to fax them directly to us at: 855-312-3594

Our front desk may also request your PCP’s contact information and details about other specialists you see.
